Pre-Return To Work Form With Microsoft Forms

As part of our measures to return to the office safely, we needed our staff to complete and return the 

Pre-return to Work Form as set out in the Government’s Return to Work Safely Protocol. Our COVID-19 Response Team developed an online solution for our staff to complete this form and to collate the results to reduce the administrative burden of applying the measures set out in this protocol. We would like to share the template with you to assist you to apply these measures as painlessly as possible.

The protocol outlines the steps employers must take, both before a workplace reopens and while it continues to operate to prevent the spread of COVID-19 in the workplace. In order minimise risk to workers, the protocol sets out that workers must complete and return a Pre-return to Work Form three days before going back to the workplace. This form seeks confirmation from workers that, to the best of their knowledge, they have no symptoms of COVID-19, that they are not currently not self-isolating nor awaiting the results of a COVID-19 test.

Helpfully, the Health and Safety Authority (HSA) have confirmed to us that our form meets the requirements of the protocol.

Our solution uses Microsoft Forms and is simple to setup and use. You should not need any specialist assistance. Once an employee fills out the form and confirms the information to be accurate and correct, it is returned to you via Microsoft Forms. Microsoft Forms will provide the detailed responses by individual together with a dashboard showing all the results. This solution is GDPR compliant as the information submitted is stored within your own organisation’s Office 365 environment.  

Microsoft Forms is available as part of the following Office 365 Plans:

  • Microsoft 365 Business Basic (formerly Office 365 Business Essentials)
  • Microsoft 365 Business Standard (formerly Office 365 Business Premium)
  • Office 365 E1, E3, and E5

Click here to get the template. This link will bring you to a Microsoft Forms web-page, follow the instructions on screen and click “Duplicate” at the top of the page. At this point, the form will be saved to your Office 365 environment and you can start to make any changes you need to make it applicable to your organisation before you circulate it.
